We must receive and review all prior Obstetric and prenatal records in order for you to be seen by Dr. Aziz. Please note your visit will be Cancelled if we do not have complete records.
If you would like to see Dr. Aziz for second opinion, please bring your prior records with you.
We respectfully request that you cancel appointments with 24-hour notice. We appreciate early notification of your change of plans so that we may offer that appointment to another patient in need. We will be happy to assist you in rescheduling your appointment for another date/time. Cancellation of an appointment with less than 24 hours notice or any appointment missed without prior notification may be subject to a $25 cancellation charge. After three missed appointments, the scheduling of future appointments will be at the discretion of your physician.
Co-payment is collected from you at the time of service. If the required payment is not made at the time of service. a $10 billing fee will be charged to your account. Payments that do not clear the bank and or are not credited to our account for any reason will be charged a processing fee of $5.00 and a bank fee of $25.00.
Certain procedures, such as IUDs require insurance pre-authorization. Please contact your individual insurance carrier to verify your benefits.
Please feel free to contact our office for fee schedules, concerns, problems or difficulty with payment. We will do our best to come to mutually satisfactory payment plans that will allow us to provide uninterrupted medical care.
Laboratory Services and Radiology Imaging Services
For your convenience, we allow our patients to go to Labcorp or Quest and radiology imaging center of their choice. However, it is the patient’s responsibility to ensure the workup is obtained and our office receive the results in a timely manner. If we do not receive the results, we cannot review them and provide optimal care.
Please call during regular office hours for all medical needs including appointments, prescriptions and lab results. In order to serve your medical needs in a timely manner, you must be prepared to leave a detailed message.
Outside of normal business hours, a Doctor is on call for emergencies only. For consultations other than immediate postoperative issues or obstetrical questions, you will be directly billed a $25 consultation fee for after-hour medical advice and treatment, which is not reimbursable by your insurance.
Your Medical Records
Your medical records will be copied and forwarded with your signed request. Unless otherwise specified, we will copy the last 3 years of your records. allow at least three weeks for your request to be processed after we have received payment for this service. We will call when they are ready to be picked up.
Maryland law (Health General Sec. 4-304) allows physicians to charge patients (or the patient's "personal representative") a fee for copying medical records. The charges may be adjusted annually for inflation.
Children Who Accompany You
Even the best behaved child may have a restless day. We ask that you respect our patients and remove disruptive children to the lobby or outdoors. Please do not leave children unattended when you are in consultation with the Doctor. Unfortunately, we do not have staffing to provide babysitting services during your exam.
